Extended Care Form

Children ages 3 through 5 can participate in our Extended Care program. This program was designed for working parents who do not need our Full-Time care program, and for parents who occasionally need to drop children off early or let them stay a little later.

All children in our 3s, 4s and 5s programs need to have the following form on file. Please fill out this form and bring it in to your child's teacher at orientation.

Extended Care Form

Calvary CDC does not require children to nap, however we realize that they do need time in the day to rest.  Your child will need to bring a crib sized sheet and small blanket if they are going to stay past 2:00PM. The sheet is mandated by the state of North Carolina, and we only keep enough on hand for unexpected drop-ins. If your child attends regularly, we will keep their sheet and blanket during the week, and send it home on Friday for laundering. Your child may also bring a small "lovie" to rest with, but please only send one item.
